Any suggestions for a good contact form script?

You could use the form-mail script that is installed by Fantastico. When you have that script installed you can create forms, but you cant use them in the current location. Once the forms are created you just download them and then upload them to any location to use. You can get to fantastico via Cpanel --> Fantastico --> phpFormGenerator
